Here's what we use in Lancaster. At the bottom is the area to give a reason for the conflict, e.g., represent a co-D, represent a witness, client is family member of office staff, etc. The form gets sent to bail admin, who finds an available attorney from the court appointed list. On homicide cases, I go to the PJ before we send the form, explain the conflict, and then inform bail admin that I already spoke to the PJ about it when we send the form.
